strong concern voiced during CP calls that this requires us to distinguish between clinically-relevant delta vs binary-different.

Presume development effort is small as Joe has worked out a solution. If we don't choose his current solution, then development likely moves to a Medium.

We need evidence that servers want to implement this. There is little worry that Document Consumers want this.
