Is patient minor bug

The Is Patient a minor? check box breaks the application UX flow with a nasty bug: if you select this when creating a patent, so you are on-boarding a minor, then close the window. Then try onboard another kind of user like a practitioner. it then looses the ability to specify an address and asks “Select Guardian” for creating a practitioner. this is a bad bug because it meant I had to re-load the page to make a practitioner.

This is delegated to sprint 7 because it is not a mission critical bug
