Issue as described by client.
"It looks like demo site (Foundation) code base is never built for a load balanced environment. We are using Foundation code as our baseline for this customer, and looks like Foundation uses concurrent dictionary for site settings which is not replicated on other instances when updates are made to site settings. "

The client registered an event registry and that resolved the issue. It would be nice if this was done out of the box so load balanced clients(includes all dxp clients) building sites off of foundation don't run into issues.

A KB could be an alternative, but it would not reach many people and this is not necessarily an easy issue to identify.
