Improve tables on the Visits summary view

The tables in the visit summary view could be improved.

1. The links in the "Visits by data-entry state" table currently raise errors. These links are broken due to a syntax error and should be fixed.
2. In the same table, if there are no visits for a category, a 0 should be displayed. For example, in the attached screenshot, In Progress columns should read 0.
3. In the same table, a "Total" row at the bottom should be included that displays the total numbers for each column.
4. In the same table, a "Total" column at the right should be included that displays the total numbers for each row.
