We should re-use peer heaps between sub channels. #199

Specifically for any sub channel created by the HyperbahnClient they should use a shared peer heap.

This will reduce the amount of memory and resources used by these sub channels.

There's been reports of production services having >22k peer heap elements probably because they have 22 sub channels.
